In short: single implants are suitable when 1 to 3 teeth are missing and the neighboring teeth are healthy. All-on-4 is indicated for rebuilding a full arch (complete tooth loss or all teeth condemned). Between the two, several alternatives exist: implant-supported bridges, All-on-6. Single implants: when it's the right choice

The single implant replaces a missing tooth with an individual titanium root, topped with a crown. It is the most common solution in routine implantology.

Typical indications:

  • Loss of a single tooth (unsalvageable decay, trauma, old extraction)
  • Missing 2 or 3 non-adjacent teeth
  • Healthy neighboring teeth (that you don't want to grind down for a conventional bridge)
  • Good bone quality, or acceptable localized graft

Advantages: each implant is independent, hygiene is identical to that of natural teeth, the esthetic result is undetectable. If an implant causes a problem in 10 years, it is replaced without touching the neighbors.

Limitations: requires enough bone at each location. For 6 missing teeth, you end up with 6 individual implants, possible but costly and surgically demanding.

All-on-4: when it's the right choice

All-on-4 was developed for fully edentulous patients or those whose entire arch of teeth is condemned. The idea: only 4 implants per jaw support a complete fixed dental arch.

The 2 anterior implants are placed vertically, the 2 posterior ones are tilted at 30-45°. This clever tilt makes it possible to avoid the areas where bone is usually resorbed (maxillary sinus, mandibular nerve), and therefore most often without a bone graft.

Typical indications:

  • Complete tooth loss (removable denture wearer, or all teeth lost)
  • All teeth condemned (terminal periodontitis, multiple unsalvageable cavities)
  • Reduced bone volume that would make single implants impossible without massive grafts

Advantages: return to fixed teeth in 1 day, removal of the denture, remarkable esthetics, total cost lower than 12-14 single implants.

Limitations: the implants are joined into one block. If an implant fails, the whole assembly must be dismantled for intervention. Hygiene requires specific tools (fine interdental brushes, water flosser).

The alternatives between the two

The implant-supported bridge (3 adjacent missing teeth)

Instead of 3 single implants, 2 implants are placed and a 3-tooth bridge is fixed on them. A good economical and reliable option for posterior partial tooth loss.

All-on-6 (a more stable variant)

6 implants per arch instead of 4. More stable, better indicated when the bone is of medium quality or the occlusion is strong. Intermediate cost between All-on-4 and 12-14 single implants.

Implant-supported removable denture ("stud-bar")

For patients who want to remove their denture to clean it in the evening: 2 to 4 implants fitted with snap buttons support a well-secured removable denture. Less esthetic but more affordable.

How to decide: the checklist

Ask yourself these questions:

  1. How many teeth am I really missing? 1-3 → single implants. 4-6 → bridges or single implants. An entire arch → All-on-4/6.
  2. Are my remaining teeth healthy in the long term? If not, is it better to extract them and switch to an All-on-4?
  3. What is my total budget? 12 single implants cost 2.5x more than an All-on-4.
  4. What is my tolerance for surgery? All-on-4 = 1 day of intervention, 12 implants = several sessions.
  5. What level of daily hygiene am I prepared to maintain?
